Sports Physical Clinic at Ocracoke Health Center

The Ocracoke Health Center is holding a sports physical clinic on Saturday, September 22.

All sports physicals for the 2012-2013 school year will be done at this time. In the past, the Ocracoke Health Center has been able to offer these sports physicals free of charge and we want to continue to offer this service. If physicals are not done at this time, they will be the responsibility of the patient/parent. The North Carolina High School Athletic Association requires that all student athletes have a physical each year.

Saturday, September 22 at Ocracoke Health Center (305 Back Road)

Please come at the appropriate time for your student’s grade and bring your completed and signed physical examination form. The physical cannot be done without this form.

8 AM 7th graders

8:30 AM 8th graders (Last Name Begins with A-O)

9:15 AM 8th graders (Last Name Begins with P-Z)

10 AM 9th graders

11 AM 10th graders

1 PM 11th graders

2 PM 12th graders
