Free School Sports Physicals

Attention: All Parents of School Student Athletes: Ocracoke Health Center is holding a free clinic for school sports physicals on May 17th-19th.

The Ocracoke Health Center is holding a sports physical clinic on Tuesday, May 17, Wednesday, May 18, and Thursday, May 19, between 3 and 5pm for all 2016-2017 sports. This will be the only clinic day to provide free physicals for the upcoming school year.

In the past, the Ocracoke Health Center has been able to offer these sports physicals free of charge. In order to be fair to scheduled patients and to continue to offer this free service, if physicals are not done at this time, they will be the financial responsibility of the patient/parent.

The North Carolina High School Athletic Association requires that all student athletes have a physical each year.

May 17 (rising 9th-12th graders), 18 (rising 6th-8thgraders), and 19 (rising 3rd-5th graders)

Please bring your completed and signed permission/physical examination form.
